KMS Camper Athlete Forms

The forms detailed below are required for all participants in any KMS Camp or Program. These forms are PDF documents that may be downloaded to your computer and opened with Adobe Acrobat. Unless otherwise noted below, please fill-in the forms on the computer and signed electronically with Adobe Acrobat and email the forms back to the appropriate contact for your camp or program. If you are unable to sign electronically, you may print and scan and email or print and deliver to the school prior to the start of the camp or program.

Note: Enrolled students will complete these forms online as part of the enrollment process and should not need to complete these specific forms below unless directed by a KMS Faculty member.

Waivers

KMS COVID-19 Acknowledgement and Release of Claims

Required by all Athletes involved in any KMS programs or camps. Please note, if you registered for a KMS Camp online, you have already agreed to this waiver. [ONLINE]

Health Information Forms

KMS Physical Health Appraisal Form - This form should be completed by parents and the student/athlete's physician. Physicians may have their own form which may, in some cases, be substituted for this KMS form. Please check with the camp contact.

KMS OTC and Prescription Medication Form - This form should be completed by parents to authorize KMS to administer Over the Counter (OTC) and Prescription medication to the student/athlete.

KMS Nutrition Information Form - This form is completed by parents and is used by the Kitchen staff. It details any food allergies and dietary requirements for the student/athlete.

Consent and Authorization Forms

KMS Treatment Consent and Medical Information Release [PDF] [ONLINE] - This form is required for all athletes involved in any KMS Camps or Programs. It authorizes KMS to treat students/athletes for minor injuries or take students to emergency care facilities and release medical information about student/athletes to those qualified medical professionals in order to treat the student/athlete.

KMS Photo Release - We ask that all parents of minors and adults involved in any KMS Camp or Program to sign this release so that any pictures or video taken by KMS Faculty/Staff may be used on our website or social media posts promoting the program or school. Please note, if you registered for a KMS Camp online, you have already completed this Release form.

Policy Acknowledgement Forms

KMS Drugs and Alcohol Policy Acknowledgement - This form summarizes the Drugs and Alcohol Policy for all student/athletes enrolled or registered for any KMS Camps or Programs. Both parents and athletes must read and acknowledge their understanding of this policy. This form may be signed by the parents before the camp however, the student/athlete must sign and acknowledge the policy in the presence of a KMS Faculty member during check-in for any camp or program.
